At Journey Way, we’re excited to walk alongside you in your recovery journey. Applying is simple — just follow the steps below.

How to Apply​

1. Review the Resident Handbook
Read through our Resident Handbook to learn about our expectations, structure, and community guidelines.

2. Complete the Application

Choose the option that works best for you:

- Simply scroll down to our digital residency application form below and submit it directly through the website.
 

*Option B: Download the file to fill it out on your device or print. Once completed, you can submit it by:

Resident Rights at Journey Way

As a resident of Journey Way, you are entitled to the following rights, which our staff is committed to upholding throughout your stay:

1. Humane Care - To receive care in an environment that supports your recovery and overall well-being.

2. Freedom from Abuse - To be free from verbal, emotional, physical, or sexual abuse.

3. Respect & Dignity - To be treated with dignity, compassion, and respect at all times.

4. Personal Choice - To choose your own recovery goals and participate actively in your recovery plan.

5. Service Expectations - To expect required services to occur during scheduled times and in designated locations.

6. Continuity of Care - To receive consistent care, including clear schedules of services and staff availability.

7. Informed Consent - To receive full information about services and give informed consent prior to beginning your stay.

8. Cost Transparency - To be informed of any associated costs, as well as the potential benefits and risks of participation in the program.

9. Confidentiality - To have your records kept private and accessible only to authorized staff, unless otherwise permitted or required by law.

10. Referral Upon Discharge - To be referred to appropriate services upon discharge or transfer.

11. Personal Property - To retain personal belongings that do not pose a risk to yourself or others.

12. Mail Privacy - To send and receive unopened mail. You may be asked to open received mail in the presence of staff when requested.

13. Private Medical Care - To be seen by a private physician, understanding that all associated costs are your responsibility.

14. Complaint Process - To file a complaint directly to the homeowner without fear of retaliation and to have the complaint addressed in a timely manner.

15. Notice of Changes - To be informed of any changes to these rights, responsibilities, program policies, or schedules.

16. Know Your Providers - To be informed of the qualifications of staff members involved in your care.

17. Right to Leave - To refuse to continue services and be informed of the consequences of leaving.

18. Support Following Dismissal - To request referrals to other resources if you are dismissed from the program.

19. Voluntary Participation - Not to be required to perform tasks or services for Journey Way that are outside of normal resident expectations.

By signing below, I certify that I have been informed of my rights as a resident of Journey Way, and that I have received, read, and understood the Journey Way Rules, which I agree to fully abide by for the duration of my residency. I further certify that all information provided in this application is true and complete to the best of my knowledge, and I understand that providing false or misleading information may result in denial or termination of my residency.
